Service Description

The service enables citizens and residents to modify their information in the civil record, including date of birth, name, permanent address, surname modification and addition, issuance of a death certificate after one year, and profession modification (for residents only).

  1. 1. To modify the names:
  2. - The concerned person must be present in person.
  3. - Copy of the personal documents of the person concerned or parents who want to change the information according to them.
  4. 2. To modify the permanent address:
  5. - The concerned person must be present in person.
  6. - A written request from the concerned person to the director general, that includes the family information.
  7. - Fill out a form prepared for this purpose approved by the Sheikh and Wali of the wilayah.
  8. - Copy of the ownership of the house or the lease contract.
  9. - Certificates of children.
  10. - Letter from the workplace.
  11. - Copy of the electricity and water meter bill.
  12. 3. To modify/add a surname:
  13. - The concerned person must be present in person.
  14. - A written request from the concerned person to the director general, that includes the family information.
  15. - Copy of the father's documents.
  16. - If the information to be modified contradicts the father's data, the concerned person should visit the Ministry of Interior to address the matter with the appropriate authority.
  17. 4. To Obtain a death certificate after more than a year:
  18. - The person who reported the death must be present and s/he must be a first-degree relative of the deceased.
  19. - Notice from the health institution where the death occurred.
  20. 5. To modify a profession for non-Omani:
  21. - The concerned person must be present in person.
  22. - Letter from the workplace stating the new job title.
